Patient Charts & Navigation
Overview
Patient charts provide a comprehensive view of patient information, tasks, episodes, and clinical documentation. The patient chart interface uses a three-column layout that adapts to your screen size, making it easy to navigate between patients, tasks, and forms.
Accessing Patient Charts
From Dashboard
- Click "View Patients" quick action card
- Or click "Patients" in the top navigation
- Navigate to
/pin your browser
From Navigation
- Use the top navigation bar → Click "Patients" icon
- Use Command Palette (⌘ K) → Search "View Patients" or "Patients"
Layout Overview
The patient chart interface uses a three-column layout:
Left Panel: Patients List
- Shows: List of all patients in your care
- Features: Search, filter, and select patients
- Responsive: Collapses on mobile, accessible via toggle button
Center Panel: Tasks & Content
- Shows: Patient tasks, forms, and content based on selection
- Features: Task list, episode selector, form navigation
- Main Content Area: Where you view and complete documentation
Right Panel: Patient Chatbot
- Shows: AI assistant for patient-specific questions
- Features: Ask questions about patient care, documentation requirements
- Responsive: Collapses on mobile, accessible via toggle button
Navigating Patients
Patient List
- View Patient List: Left panel shows all patients assigned to you
- Search Patients: Type in search box to filter by name
- Select Patient: Click on a patient to load their chart
- Patient Information: Each patient card shows basic info (name, status)
Patient Selection
- Click Patient: Opens patient chart in center panel
- Patient Info: Shows in center panel header
- Tasks Load: Patient tasks appear below patient info
- Episode Display: Current episode information shown
Understanding Episodes
Episode Concept
Patients can have multiple episodes of care:
- Each episode represents a distinct period of care
- Episodes have start and end dates
- Tasks are organized by episode
- You can switch between episodes for the same patient
Current Episode
- Default View: Shows the most recent or current episode
- Episode Badge: "Current" badge indicates active episode
- Episode Info: Shows start date and task count
Switching Episodes
- Access Episode Selector:
- Click dropdown menu (three dots) on task list
- Select "Switch Episode"
- View Episodes: List shows all episodes with dates
- Select Episode: Click episode to switch view
- Create New Episode: Option to create new episode from menu
Viewing Tasks
Task List
The center panel displays tasks for the selected patient and episode:
- Task Cards: Each task shown as a card or row
- Task Information:
- Task type (e.g., "SOC Visit", "OASIS Assessment")
- Visit date (if scheduled)
- Status indicators
- Completion status
- Task Actions: Options to view, edit, or delete tasks
Task Status
Tasks show different status indicators:
- Not Started: Task not yet begun
- In Progress: Task partially completed
- Completed: All forms in task completed
- Overdue: Task past due date (if applicable)
Task Types
Different task types require different documentation:
- SOC (Start of Care): Initial visit documentation
- Recertification: Periodic recertification visits
- Routine Visits: Standard patient visits
- Assessments: Clinical assessments (OASIS, etc.)
- Custom Tasks: Organization-specific task types
Accessing Forms
From Task List
- Click Task: Opens task detail view
- View Forms: See table of contents showing all required forms
- Select Form: Click on a form name to open it
- Navigate Forms: Use arrows to move between forms
Form Table of Contents
When viewing a task, you'll see:
- Form List: All forms required for the task
- Form Names: Display names for each form type
- Navigation Arrows: Quick access to open forms
- Progress Indicators: Visual feedback on completion
Form Navigation
- Previous/Next: Use arrows in form header
- Form List: Click form name in table of contents
- Keyboard: Arrow keys for navigation (where supported)
- Back Button: Return to task view from form
Patient Information
Basic Information
Patient chart header shows:
- Patient Name: Full name displayed prominently
- Patient ID/Referral ID: Unique identifier
- Current Episode: Episode dates and info
- Status: Patient status indicators
Additional Patient Details
- Demographics: Access via patient info panel
- Insurance: Insurance information
- Contacts: Emergency contacts and family
- Medical History: Relevant medical information
- Allergies: Known allergies and reactions
Task Management
Creating Tasks
- Access Create Task:
- Click "+" button or "Create Task" option
- Available from task list view
- Select Task Type: Choose from available task types
- Select Visit (if applicable): Link task to scheduled visit
- Set Due Date (optional): Assign target completion date
- Create: Task created with required forms
Editing Tasks
- Task Menu: Click three dots (⋯) on task
- Edit Details: Modify task information
- Change Visit: Link to different visit
- Update Status: Mark as complete or change status
Deleting Tasks
- Task Menu: Access via three dots
- Delete Option: Select delete (requires confirmation)
- Confirmation: Confirm deletion (cannot be undone)
- Cleanup: Associated forms handled appropriately
Tips & Best Practices
Navigation
- Use Keyboard: Learn keyboard shortcuts for faster navigation
- Panel Toggles: Use panel toggle buttons to maximize workspace
- Search First: Search for patient before scrolling
- Episode Awareness: Know which episode you're viewing
Task Management
- Complete in Order: Follow task sequence when possible
- Check Requirements: Ensure all required forms are completed
- Review Before Submit: Review forms before marking complete
- Use Episodes: Organize care by episodes appropriately
Efficiency
- Batch Similar Tasks: Group similar tasks when possible
- Use Templates: Leverage form templates for consistency
- Keyboard Shortcuts: Learn and use keyboard navigation
- Patient Chatbot: Ask questions to clarify requirements

Mobile Layout
On mobile devices, the three-column layout becomes:
- Single Panel View: One panel visible at a time
- Toggle Buttons: Access other panels via buttons
- Swipe Gestures: Where supported, swipe between panels
